Send an official transcript to CGCC
Please contact your former institution to request an official transcript be sent directly to the CGCC Records Office:

A hand-carried transcript cannot be accepted for an official evaluation.
Seek Academic Advisement
Bring unofficial transcripts and meet with an advisor for a transcript evaluation.
Take Assessment/Placement Test
Your advisor may determine that you need placement testing, which should take place prior to registering for classes.
Register for Classes
Submit Authorization for Official Transcript Review
Transfer student may not submit for an official transcript evaluation until they have registered for classes.
IMPORTANT NOTE:
You must complete and submit a Prior College Credit evaluation Form, either in person of by fax*, for your transcripts to be evaluated.
* Please allow 48 hours for fax processing

Students must file a transcript request through CGCC Records. CGCC will not automatically send transcripts, nor will a university request them for the student. Remember, it is a good idea to keep a copy of your transcripts for your records, however only transcripts sent directly from CGCC to another institution are considered OFFICIAL. The cost of each official transcript is $5.00.
Four ways to request transcripts from CGCC:
- Online: at my.maricopa.edu
- In person: Fill out the transcript request form in the Admissions & Records office in the AGA Building. 480.732.7320
- By mail*: Mail a written and signed request to:

* The written request must include your:
- Full Name
- Student ID Number
- Date of Birth
- Dates of Attendance at CGCC
- Name and address of the institution where the transcript need to be sent
Official transcripts will not be issued to students having outstanding debts to any of the Maricopa Community Colleges. The release of transcripts is governed by the guidance of the Family Education Rights and Privacy Acts of 1974.
