2011-12 Financial Aid Forms
11-12 Dependent Verification Worksheet
This form, along with copies of student and parent 2010 Federal Tax Returns, is used to verify the information on the student's financial aid application and determine if corrections are needed due to the student's application being selected for verification by the Department of Education.
11-12 Independent Verification Worksheet
This form, along with copies of student/spouse 2010 Federal Tax Returns, is used to verify the information on the student's financial aid application and determine if corrections are needed.

11-12 Dependent Household and Untaxed Income Worksheet
This form is used to verify the information on the student's financial aid application and determine if corrections are needed due to the student's application being selected for verification by the Department of Education.  The student and parent must complete the form. 
11-12 Independent Household and Untaxed Income Worksheet
This form is used to verify the information on the student's financial aid application and determine if corrections are needed due to the student's application being selected for verification by the Department of Education.  The student and spouse (if married) must complete the form. 
11-12 Asset Verification Form
This form is needed when asset information for student and/or parent has been left blank on the financial aid application. It is also necessary in some cases, to clarify business and asset net worth.

11-12 Homeless Determination Request Form
This form is needed in order to evaluate your current living arrangements and proceed with determination of your eligibility for federal student aid.
11-12 Loan Discharged Due to Disability Form
This form is needed because our office has been notified you have one or more prior student loans discharged due to total and permanent disability. 
11-12 Request for Additional Loan Funds Form
At CGCC, we do not award the maximum in student loans upfront.  We expect students to carefully review their budget before borrowing additional funds.  This form assists students in reviewing their borrowing history and is needed in order for CGCC to award any additional loan funds. 
11-12 Request for Income Reduction
This request can be submitted to the CGCC Financial Aid Office when a student and/or parent has had a reduction in their income from the information required on the FAFSA to the next tax year.

Non Year Specific Forms
Consortium Agreement
This form is needed when a student is taking classes at another Maricopa Community College, Arizona State University or Central Arizona University and the student would like to receive their aid based on those credit hours in addition to their credit hours at CGCC. Before submitting the agreement to the financial aid office student's must enroll at the participating school and pay for the classes at the participating school. There are deadline dates each semester to submit a consortium agreement. Please check with the CGCC Financial Aid Office for these dates. Note: In order to receive a student loan at CGCC, the student must be enrolled in a minimum of 6 credit hours at CGCC.  In addition, we do not honor consortium agreements in the summer semester(s). 

Revision Form
This form may be necessary when a student changes their enrollment status, wants to cancel their financial aid, needs to request a student loan, and other miscellaneous requests.

Satisfactory Academic Progress Appeal
This form needs to be submitted when a student has been denied financial aid due to their academic progress and they wish to appeal.

Maximum Timeframe Appeal
Students must complete their academic objective in an allowable timeframe. Once a student has reached this timeframe (i.e. already have completed a degree, have attempted excessive credit hours) they will need to complete this form in order for the Financial Aid Office to determine if they are eligible to receive financial aid at CGCC.
