Application for Education Benefits.
VA Form 22-1905, Authorization and Certification of Entrance or Reentrance into Rehabilitation and Certification of Status. This form is completed by the Vocational Rehabilitation case manager, not the veteran.
Pay Request/Tuition Deferment Form - This request must be completed each semester.
